چکیده

Strategic plans provide the roadmap by which organizations achieve their vision. To effectively serve as that roadmap, strategic plans must have certain essential characteristics. These include the ability to inspire and motivate while remaining action-oriented and understandable to all personnel. More than a decade ago, in Crossing the Quality Chasm, the Institute of Medicine (IOM) suggested organizing transformational efforts in 6 domains: Safety, Effectiveness, Patient-Family Centeredness, Timeliness, Efficiency, and Equity. Recently 2 additional domains have been added: Access and Care Coordination. Since that time, many quality and safety efforts and hospital strategic plans have been built around those domains. Although the IOM structure is succinct, the domains important, and the strategic vision thorough, it did not inspire or motivate our staff sufficiently to be the framework for change in our hospital. Therefore, Nationwide Children’s Hospital developed a strategic plan, organized entirely from the point of view of the patient and family. We believe this quality and safety strategic plan is readily understandable by all, more motivational because of its direct link to the patient, and offers an effective alternative to the IOM domains of transformative strategic thinking (Table 1).

Crossing the quality chasm in neonatal-perinatal medicine.

The "Quality Chasm" exists in neonatal intensive care. Despite years of clinical research in neonatology, therapies continue to be underused, overused, or misused. A key concept in crossing the quality chasm is system redesign. The unpredictability of human factors and the dynamic complexity of the neonatal ICU are not amenable to rigid reductionist control and redesign. Change is best accompli...

How can primary care cross the quality chasm?

The chasm between knowledge and practice decried by the Institute of Medicine (IOM) is the result of other chasms that have not been addressed. They include the chasm between what we know and what we need to know to improve care; the chasm between those who provide primary care and those who do not fund, study, support, or publish practical primary care studies; and the chasm between research a...

Crossing the quality chasm in resource-limited settings

Over the last decade, extensive scientific and policy innovations have begun to reduce the "quality chasm"--the gulf between best practices and actual implementation that exists in resource-rich medical settings. While limited data exist, this chasm is likely to be equally acute and deadly in resource-limited areas. While health systems have begun to be scaled up in impoverished areas, scale-up...
